Dear Parents/Guardians,
The Youth Artist Program (YAP) has organized a field trip for Saturday, March 14, 2020, to the Pennsylvania Academy of Fine Arts. This Field trip is for up to 10 participants on a first come first serve basis. Please return this permission slip quickly to ensure your child’s reservation.
Please keep this sheet for reference and return the latter with the signature before the trip.
*If any changes occur before the journey, parents will be contacted immediately.

Conditions of participation are that every student agrees to abide by the rules governing this activity, to cooperate with the teacher and staff, and to obtain a liability waiver from his/her parent or guardian.
Every effort will be made to ensure proper safety conditions and supervision. The Taller Puertorriqueño and The Pennsylvania Academy of Fine Arts will not accept legal responsibility for accidents or injuries.
